There are three simple steps to ensure your team's entry into the tournament:

Please include your
(A) team name(s)
(B) the division(s) in which you want your team(s) to play
(C) a name & phone number where you can be reached for questions
2) Submit your team's Official Roster
Submit your roster using the form below. Remember, you can enter your reservation without your roster until May 17th, 2011
3) Submit your team's $450.00 Entry Fee
Submit your check or money order payment by mail. We must receive your payment no later than May 17th, 2011.
